About this role

Yea and District Memorial Hospital is seeking applications for a District Nurse - Registered Nurse 0.4 EFT (32 hours per f/n). This role represents an exciting opportunity for a dedicated District Nurse to join the YDMH team. We strive to deliver safe, high quality care to the local community.

The successful applicant will be responsible for providing services to the local community in the comfort of their home. The ideal candidate will collaborate with our clinical workforce to deliver nursing services and support. This position reports to the Director of Clinical Services.

Join a team focused on community-based care under the Nurses and Midwives (Victorian Public Sector) Enterprise Agreement 2024-2028. Classification ranges from YQ1 to YQ3 dependent on experience. Applications close 31/12/2025.

Requirements

Registered Nurse qualification
Classification YQ1 – YQ3 dependent on experience
Current AHPRA registration
Experience in community or district nursing
Ability to work independently in home environments
Victorian drivers licence
Knowledge of Victorian public sector nursing practices

Responsibilities

Provide nursing services to the local community in the comfort of their home
Deliver safe, high quality care during home visits
Collaborate with clinical workforce to support community nursing needs
Assess and manage patient conditions in community settings
Administer treatments and medications at patients' homes
Coordinate care for chronic disease and palliative patients
Report to the Director of Clinical Services
