About this role
Presbyterian Homes & Services is hiring a RN Telephone Triage Nurse to provide after hours and weekend telephone triage support to Assisted Living Communities. This position will office out of one of our PHS locations and/or from home. It requires a current RN license for Minnesota, Wisconsin and Iowa.
The Telephone Triage Nurse is responsible for providing telephone response and delegation to Resident Assistants working within Assisted Living settings. Competencies include clinical assessment, computer skills and customer service. The role supports regulatory and clinical practices through over-the-phone assessment to ensure high quality care for residents.

Requirements
- Registered nurse with current licensure in Minnesota, Wisconsin, and Iowa
- Interest in gerontological nursing, rehabilitative and restorative clinical practices
- Assisted Living nursing and/or phone triage experience preferred
- Demonstrated compatibility with PHS's mission and operating philosophies
- Demonstrated ability to read, write, speak and understand English
- Competencies in clinical assessment, computer skills, and customer service
Responsibilities
- Provide after hours and weekend telephone triage support to Assisted Living Communities
- Deliver telephone response and delegation to Resident Assistants (Home Health Aides)
- Conduct clinical assessment over the phone for residents
- Support regulatory and clinical practices in providing direction
- Ensure high quality care through over-the-phone clinical assessment
- Utilize computer skills for documentation and triage processes
- Demonstrate customer service in interactions with residents and staff
