As a Triage Nurse (NOT REMOTE), you assist patients’ needs over the phone, provide medical advice within your scope of practice, coordinate care, and determine urgency to ensure appropriate care. You help patients and families navigate difficult times with support and guidance after normal operating hours.
You deliver telephonic nurse assessments, advice, and instructions to patients and caregivers, communicate changes to case managers and physicians, and assign nursing visits accordingly. Work closely with patients on a 1-1 level while receiving strong support from a patient care team including nurses, social workers, chaplains, volunteers, and CNA leadership.

Requirements
- Current LVN/LPN licensure (in good standing) in the state(s) of practice with one year of experience under license
- Current CPR Certification
- Previous hospice experience preferred
- Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
- Work closely with patients on a 1-1 level but get strong support from a patient care team
- Can work independently on a day-to-day basis with phone, email, written communication, meetings with healthcare professionals, and understanding of hospice rules and regulations
- Ability to learn an electronic medical records system
- Reliable Transportation, Driver's License, Car insurance
Responsibilities
- Deliver telephonic nurse assessment, advice, and instruction to patients and caregivers after normal operating hours within scope of practice
- Provide case manager and management team with information regarding changes in patient’s condition after telephonic contact
- Provide health care instruction to patient/caregiver in a clear and concise manner as appropriate per assessment and plan of care
- Teach patient and family/caregiver self-care techniques as appropriate
- Communicate with physician regarding patient’s needs, report changes in condition, and obtain/receive physicians’ orders as required
- Assign nursing visits accordingly
