Serve as the primary point of contact for patients with health or illness questions via telephone. Provide remote nursing care to diverse populations, including infants, elderly, behavioral health needs, and recently discharged patients. Perform skilled assessments, prioritize needs, and refer to appropriate care levels.
Utilize evidence-based guidelines, nursing process insight, and critical analysis of patient information and EMR for decision-making. Ensure safe, timely disposition of calls by assigning acuity levels from emergent to non-acute. Document encounters concisely using patient words and educate based on active listening principles.

Requirements
- Registered Nurse license
- Reside within an hour of Cleveland, Ohio
- Proficiency accessing and documenting in EMR
- Skilled in thorough nursing assessments across life span populations
- Ability to prioritize patient needs and refer to appropriate care levels
- Experience using evidence-based guidelines and nursing process
- Critical analysis of patient information for decision-making
Responsibilities
- Serves as the primary point of contact for telephone patients with a variety of health or illness questions
- Collects subjective data related to the presenting problem and medical history
- Utilizes critical thinking and problem-solving skills to recognize and match symptoms to protocol, assigning acuity and ensuring safe, timely disposition
- Offers home treatment advice as appropriate and accesses patient information via EMR
- Documents encounter and decision making in EMR in a concise and accurate manner, utilizing the patient's own words
- Educates patients utilizing principles of active listening and communicates based on patients' level of understanding
- Recalls knowledge of teaching principles of health care information to pediatric, adult and geriatric populations
