About this role

Vancouver Coastal Health implements minimum Nurse to Patient Ratios (mNPRs) to enhance nursing practice and quality of care. As a Mental Health Nurse at the Crisis Home Venture team, provide individualized direct nursing care and brokerage services to clients with severe mental illness, concurrent disorders or dementia. Enhance quality of life from the client/family perspective.

Perform physical and psychosocial rehabilitation assessments while consulting with health care givers and family to develop strength-based treatment plans. Establish and maintain therapeutic relationships based on respect. Administer prescribed medicines, monitor responses and educate on daily living, medication use and side-effects.

Venture is a short-term crisis intervention program offering an alternative to hospitalization for mental health and substance use clients. The 24-bed facility provides 24-hour nursing care, daytime psychiatry, GP services and recreation therapy to facilitate community transitions. Work independently and collaboratively in this Vancouver Community setting.

Practice within client and family centred care model per BCCNM standards and VCH policies. Participate in team meetings, quality improvement and maintain knowledge updates. Guided by values of caring for everyone, continuous learning and better results, thrive in quality practice environments.

Requirements

  • Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with BCCNM
  • Diploma in Nursing or Psychiatric Nursing plus two years’ recent related experience with severe psychiatric illness
  • Valid BC Driver’s license with local area travel requiring use of a vehicle as needed
  • Knowledge of principles and practices of client and family centered recovery model in mental illness
  • Ability to work independently and as a member of a multidisciplinary team
  • Skills in psychosocial rehabilitation assessments and strength-based treatment planning
  • Competence in administering medications and monitoring responses in mental health context
  • Experience with clients having concurrent disorders and/or dementia

  • Perform physical and psychosocial rehabilitation assessments
  • Consult, confer and collaborate with health care givers and family to develop, implement and modify treatment plans
  • Establish, maintain and enhance therapeutic relationships based on respect
  • Administer prescribed medicines and monitor client response
  • Demonstrate and provide information on activities of daily living, medication use and side-effects
  • Prepare and maintain client records according to established standards
  • Participate in team meetings, planning and quality improvement activities
