About this role
Northpointe Behavioral Healthcare System is a Community Mental Health organization serving individuals with serious mental illness, emotional disturbance, substance abuse disorders, and intellectual/developmental disabilities across Iron, Dickinson, and Menominee counties in Michigan's Upper Peninsula. Join as a passionate, driven Community Nurse on full-time hourly basis requiring on-the-job travel. Provide seamless, patient-centered care in Adult Foster Care group homes and Behavioral Health Home program.
The primary function involves nursing services through person-centered planning, including medication monitoring, assessment, planning, implementation, and evaluation for those at risk or experiencing psychiatric/physical symptoms. Services vary with medical orders to address physical, behavioral, and social needs of vulnerable individuals. Focus on improving outcomes, access, and efficiency within the community.
Work as part of a multi-disciplinary integrated care team with Residential Home Managers and treatment staff. Actively participate in home staff meetings for effective communication and collaboration. Coordinate health care services, attend medically necessary appointments, and provide consultation on health-related issues.
Educate individuals and staff on health and medications using current best practices. Maintain knowledge through required continuing education and seek opportunities to expand expertise. Train staff on medication administration, care plans, protocols like seizure management, and emergency response.
Requirements
- Licensed as a Registered Nurse or Licensed Practical Nurse
- Experience providing care to individuals with serious mental illness and intellectual/developmental disabilities
- Knowledge of psychiatric and physical symptom assessment and management
- Proficiency in person-centered planning and medication monitoring
- Ability to collaborate in multi-disciplinary integrated care teams
- Skills in staff training for health protocols and emergency response
- Familiarity with community mental health services and residential care settings
- Commitment to completing continuing education for current nursing knowledge
Responsibilities
- Provide nursing services through person-centered planning practices including medication monitoring, assessment, planning, implementation, and evaluation
- Work effectively with Residential Home Managers and treatment team by participating in home staff meetings
- Educate individuals and staff related to health and medications using current information and best practice guidelines
- Coordinate health care services for individuals and attend all medically necessary appointments
- Complete follow-up documentation on medical orders and train residential staff to manage health emergencies
- Identify health/nursing care needs and implement care plans/procedures under direction of Integrated Care Team Lead
- Train staff and oversee medication administration process within residential services
- Review medication and fall-related incident reports and provide consultation to residential care staff
