About this role

Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients’ homes. Deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting. Focus on complex needs like tracheostomy and ventilator support.

Provide trach/vent care, medication administration, and catheter care while monitoring vital signs and documenting conditions. Assist with daily living activities and tasks requiring hand use for clients like a bright 24-year-old woman who enjoys drawing and crafting. Support general personal care in family homes with involved parents.

Work with vibrant clients such as a fully cognitive 24-year-old in Jerome, MI, or a fun 18-year-old in Hillsdale, MI, who loves cards, movies, and bonfires. Shifts include days like 7:30am-5:30pm Tuesdays/Fridays or overnights like 10:30pm-8:30am. Homes feature friendly dogs and non-smoking environments.

Requirements

  • Active RN or LPN license in the state of Michigan
  • Home care or private duty experience preferred (not required)
  • Trach/vent experience a plus (training provided)
  • Strong communication and critical thinking skills
  • Comfortable providing care to pediatrics (birth–21 years) and adults with complex medical needs

Responsibilities

  • Provide skilled nursing care including trach/vent care, medication administration, and catheter care
  • Monitor and document patient condition and vital signs
  • Assist with daily living activities (ADLs)
  • Communicate with families and care teams
  • Maintain safe, high-quality, patient-centered care
  • Support personal care tasks requiring hand use
  • Deliver 1:1 care to pediatric and adult patients with complex needs
