About this role
Northpointe Behavioral Healthcare System is a Community Mental Health organization providing services to individuals with serious mental illness, emotional disturbance, substance abuse disorders, and intellectual/developmental disabilities across Iron, Dickinson, and Menominee counties in Michigan's Upper Peninsula. The Community Nurse role offers full-time hourly employment requiring on-the-job travel. This position delivers seamless, patient-centered care for physical, behavioral, and social needs to vulnerable individuals in Adult Foster Care group homes and the Behavioral Health Home program.
The Community Nurse provides nursing services through person-centered planning practices, including medication monitoring, assessment, planning, implementation, and evaluation for those at risk or experiencing psychiatric/physical symptoms. Services vary with medical orders and focus on integrated care within the community. The role works as part of a multi-disciplinary integrated care team to improve outcomes, access, and efficiency.
Daily collaboration occurs with Residential Home Managers and treatment team members through active participation in home staff meetings for effective communication. The nurse coordinates health care services, attends medically necessary appointments, and ensures staff training on health emergencies and protocols like seizure management. Consultation is provided to residential staff on health-related issues.
Ongoing professional development involves maintaining current knowledge via required continuing education hours and seeking opportunities to expand the knowledge base. The role oversees medication administration, reviews incident reports for interventions, and assists with enhanced pharmacy benefits and durable medical equipment. This supports comprehensive care in residential settings.
Requirements
- Licensed Registered Nurse in Michigan
- Knowledge of person-centered care planning for psychiatric and physical symptoms
- Experience providing care to individuals with serious mental illness and intellectual/developmental disabilities
- Ability to collaborate with multi-disciplinary teams in community mental health settings
- Proficiency in medication monitoring and administration oversight
- Familiarity with integrated care coordination and health protocols like seizure management
- Current continuing education in nursing practices
- Valid driver's license for on-the-job travel across counties
Responsibilities
- Provide nursing services through person-centered planning practices including medication monitoring, assessment, planning, implementation, and evaluation
- Work effectively with Residential Home Managers and treatment team by actively participating in home staff meetings
- Educate individuals and staff related to health and medications using current information and best practice guidelines
- Coordinate health care services for individuals within the residential system and attend all medically necessary appointments
- Complete follow-up documentation on all medical orders and train residential home staff to manage health emergencies
- Identify health/nursing care needs and implement care plans/procedures under direction of the Integrated Care Team Lead
- Train staff and oversee the medication administration process within residential services
- Review residential medication and fall related unusual incident reports and determine necessary interventions
