We are seeking a Geriatric Psychiatry Nurse Practitioner to join the team and provide psychiatric consultations for patients with cognitive impairments and behavioral issues. Bloom cares for a complex geriatric population where many patients, families, and caretakers face various forms and stages of dementia.

The role centers on delivering consultations with Bloom PCPs, conducting psych evaluations, and recommending changes to care plans. The nurse practitioner also evaluates psychiatric medication use and provides ongoing support for the most complex patients with dementia.
