About this role
The MedServ Group is a rapidly growing medical practice that provides top-of-the-line care to residents in a nursing home setting. We are seeking a compassionate and experienced Primary Care Nurse Practitioner to provide in-person primary care services within a nursing home and long-term care setting.
This role focuses heavily on patient interaction, counseling, education, chronic disease management, and building meaningful relationships with residents and families. The practitioner spends significant time communicating with patients, families, and caregivers regarding health concerns, goals of care, medications, and treatment options.
Daily work includes providing comprehensive primary care to residents in a skilled nursing or long-term care facility. Additional duties involve educating residents and families on disease prevention, medication adherence, nutrition, and lifestyle modifications while documenting encounters accurately in the electronic medical record.
The position offers part-time hours typically scheduled from 9AM to 5PM with 7-8 hours dedicated to visiting patients plus paperwork. The MedServ Group covers medical malpractice insurance for the role.
Requirements
- Current Nurse Practitioner license in the applicable state.
- Board certification as Family NP (FNP), Adult-Gerontology NP (AGNP), or related specialty preferred.
- DEA license preferred.
- Experience in geriatrics, long-term care, hospice, primary care, or nursing home settings preferred.
- Strong communication and counseling skills.
- Compassionate bedside manner and patient-centered approach.
- Comfortable working independently while collaborating with interdisciplinary teams.
Responsibilities
- Spend significant time communicating with patients, families, and caregivers regarding health concerns, goals of care, medications, and treatment options.
- Provide comprehensive primary care to residents in a skilled nursing or long-term care facility.
- Educate residents and families on disease prevention, medication adherence, nutrition, and lifestyle modifications.
- Document encounters accurately in the electronic medical record (EMR).
- Manage chronic conditions and support goals of care discussions during visits.
- Handle 15-20 patients per visit during scheduled facility rounds.
