The field-based Population Health Advanced Practice Provider completes patient visits across the territory with emphasis on gap closure and discovery of unidentified complex conditions. Technology-enabled chart review supports identification of clinical gaps while HEDIS and STAR metrics guide daily priorities. Visits occur on existing Bloom Primary Care panels rather than an independent caseload.

Providers collaborate daily with internal clinicians and external partners using population health data at the practice level. The Sr. Director of Population Health offers direct support for identifying improvement opportunities and executing process changes. Ongoing education keeps the team current on accountable care performance and home-based medicine.

Requirements

  • Licensed Nurse Practitioner or Physician Assistant
  • Experience with HEDIS measures and STAR ratings
  • Proficiency using technology-enabled chart review tools
  • Ability to interpret and act on population health data
  • Strong relationship-building skills with care teams and external partners

Responsibilities

  • Complete patient visits across Bloom territory focused on gap closure and discovery of previously unidentified complex conditions
  • Use technology-enabled chart discovery to highlight clinical gaps and improve outcomes
  • Support HEDIS gap closure and STAR rating success across assigned practices
  • Engage with and appropriately utilize population health data to drive superior patient outcomes at the practice level
  • Identify opportunities and execute process improvement plans with support from the Sr. Director of Population Health
  • Demonstrate exceptional communication and relationship-building with internal clinicians and external partners
  • Attend ongoing educational opportunities to maintain current knowledge in population health
