About this role

This role provides clinically based, risk reducing care and coordination that facilitates cost-effective, quality healthcare along the continuum. It manages individualized, outcome-oriented nursing care for pediatric and adult patients using the nursing process, from assessment through evaluation.

The nurse directly interfaces with physicians, care managers, health care teams, patients, and caregivers to achieve desired patient outcomes. Daily responsibilities include medication administration, vaccine administration, medication reconciliation, and initiation of patient outreach with documentation of outcomes.

South Shore Medical Center offers flexible shifts with minimal holiday and weekend requirements, along with onsite parking. Newly enhanced triaging and improved workflow processes are designed to support both staff and the patient experience. This creates a collaborative environment for focused patient care.

Requirements

  • Excellent written and verbal communication skills, with the ability to listen and assimilate complex clinical information.
  • Commitment to coaching patients on health promoting behaviors.
  • Ability to work autonomously and in teams.
  • Knowledge of and adherence to SSH policies and procedures for medication administration and documentation.
  • Safe administration of medications and vaccines.
  • Ability to develop, evaluate, and update individualized care plans.
  • Current, unrestricted Registered Nurse license in the state of Massachusetts.
  • Basic Life Support (BLS) certification from the American Heart Association.

Responsibilities

  • Manage individualized, outcome-oriented nursing care for pediatric and adult patients using the nursing process, including assessment, planning, implementation, and evaluation.
  • Administer medications and vaccines following SSH policies and procedures, including medication reconciliation.
  • Develop, evaluate, and update individualized care plans for each patient and document outcomes.
  • Initiate patient outreach and document patient interactions to support care coordination.
  • Interface directly with physicians, care managers, health care teams, patients, and caregivers to achieve desired patient outcomes.
  • Provide care, treatment, and services through coordination and completion of cross-continuum care processes.
  • Participate in quality improvement efforts, research, and evaluation processes related to patient care management.
