This role matters. At the Multnomah County Health Department, we work with communities to advance health equity, protect the most vulnerable, and promote health and wellness for everyone. We are seeking a visionary and dynamic Community Health Nurse to join our Integrated Clinical Services team, bringing skill and passion to a challenging and evolving environment.

In this role, you will provide comprehensive community and clinic health services directed towards the prevention of disease, the promotion of health, and clients' self-care. You will use professional judgment in comprehensive assessment, screening, nursing diagnosis, care planning, counseling, intervention, case management, health information, education, referral, and evaluation.

You will create a work culture of respect, trust, and understanding for the highly diverse populations the Health Department serves, both in the office and in the community. Requirements

  • Current, unencumbered Registered Nurse (RN) license in the state of Oregon.
  • Knowledge of and experience with the African American community and culture.
  • Experience leading multicultural and diverse teams.
  • Ability to perform comprehensive nursing assessments and develop individualized care plans.
  • Experience in community health, public health, or ambulatory care nursing.
  • Strong clinical judgment and ability to work independently in the field and clinic settings.
  • Proficiency with electronic health records and documentation standards.
  • Commitment to health equity and serving vulnerable populations.

Responsibilities

  • Proactively assess, plan, implement, and evaluate healthcare services that improve and promote health outcomes.
  • Provide regular screening, comprehensive assessment, and interventions for ongoing health issues such as depression, diabetes, asthma, wound and pain management, and other chronic diseases.
  • Assure completion of referrals and coordinate care with other healthcare providers.
  • Collaborate with the healthcare team to adapt care plans and ensure continuity of care.
  • Deliver health education, counseling, and referral services to clients and families.
  • Conduct community outreach and home visits to assess environmental and social determinants of health.
  • Document all patient encounters and interventions accurately and timely in the electronic health record.
  • Participate in quality improvement initiatives and interdisciplinary team meetings.
