About this role
Within an interdisciplinary team-based care model, Community Health Nurse cares for clients with acute, chronic and palliative care needs in collaboration with client/family/caregivers. Works in partnership with the client and family to establish goals that are safe, realistic, and reasonable. Assesses service eligibility and facilitates the development, implementation, and adaptation of a care plan.
Delivers direct care and provides care management to assist clients to manage their own care and navigate community services. Incorporates current evidence into practice and works in consultation with other system partners. Practices in accordance with BCCNM standards, Code of Ethics for Registered Nurses, and VCH policies.
Vancouver Coastal Health implements minimum Nurse to Patient Ratios (mNPRs) in partnership with the Ministry of Health and BC Nurses’ Union. Requirements
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
- One year recent related clinical experience in a relevant clinical area working with complex clients/patients with chronic health conditions and/or functional mobility issues including recent experience in care and transition planning
- Valid BC Driver’s License required as local area travel may require the use of a vehicle
- Demonstrated knowledge and skills in community health nursing theory and practice within a client and family centred model of care
- Demonstrated ability to provide client-focused, trauma informed, harm reduction, and culturally safe care in completing initial and ongoing client assessments and interventions
- Demonstrated ability to assess safety and risk prior to entering familiar and unfamiliar home environments
- Demonstrated ability to manage psychosocial and behavioral issues, and employ effective conflict resolution
- Demonstrated ability to manage and prioritize clients and promote cooperation, assertiveness, creative planning for change
