This Treatment and Triage Nurse role offers patients an alternative to the Emergency Room by providing telephonic and in-person triage, IV therapy, medication administration, wound care, and focused nursing assessments. It serves as an access point to the medical center, accepting walk-ins and fielding day-to-day clinical needs.

Day-to-day duties include performing clinical nursing care such as central and peripheral line care, infusions, oxygen administration, and medical emergency management. The nurse proactively engages the integrated care team to resolve barriers and ensures accessibility to services by adhering to policy.

The position operates in a professional office environment with close patient contact, requiring protective attire like gloves and masks. Collaboration with the entire integrated care team using evidence-based techniques like SBAR is essential to decreasing unnecessary ER utilization.

This role offers the opportunity to work independently with limited oversight while coaching patients on disease self-management. It is ideal for experienced RNs who thrive in a dynamic, patient-centered setting focused on reducing emergency room visits.

Requirements

  • Active, unrestricted license as a Registered Nurse in the state of practice.
  • Compact RN license preferred, or willingness to obtain.
  • 5+ years of recent clinical experience (hospital, ER, urgent care, infusion center, etc.).
  • Bachelor's Degree in Nursing preferred.
  • Active CPR certification.
  • Highly skilled with peripheral and central line care.
  • Excellent interpersonal communication skills.
  • Ability to provide nursing care independently with limited oversight and experience with socially and medically complex patients.

Responsibilities

  • Perform clinical nursing care, including telephone and in-person triage, central and peripheral line care, infusions, wound care, medication and oxygen administration, inhalation therapy, and medical emergency management.
  • Participate in quality assurance for point of care CLIA waived laboratory.
  • Ensure accessibility to medical and nursing services by adhering to policy and procedure.
  • Respond to medical emergencies and manage acute patient needs.
  • Proactively engage with the entire integrated care team to resolve barriers to care and clinical nursing services.
  • Coach and educate patients on disease self-management and appropriate utilization of healthcare services.
  • Maintain excellent communication between integrated care teams using evidence-based techniques like SBAR.
  • Take part in innovative problem solving to decrease unnecessary emergency room utilization.
