About this role

As a Community Health Nurse with Vancouver Coastal Health, you will care for clients with acute, chronic and palliative care needs within an interdisciplinary team-based model. You will work in partnership with clients and families to establish safe, realistic and reasonable goals.

Your daily responsibilities include assessing, planning, coordinating, organizing, implementing, evaluating and transitioning nursing and other services. You will assess service eligibility, facilitate care plan development and adaptation, deliver direct care, and provide care management to help clients navigate community services.

You collaborate with other system partners and incorporate current evidence into your practice. This role is based on the Community Care Clinic team in North Vancouver, within a client and family centred care model.

Vancouver Coastal Health is part of the provincial implementation of minimum Nurse to Patient Ratios (mNPRs), an initiative to enhance nursing practice and strengthen quality of care. This fosters quality practice and learning environments where nurses can grow and thrive.

Requirements

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
One year recent related clinical experience in a relevant clinical area working with complex clients/patients with chronic health conditions and/or functional mobility issues.
Recent experience in care and transition planning, or an equivalent combination of education, training and experience.
Valid BC Driver’s License required as local area travel may require use of a vehicle.
Demonstrated knowledge and skills in community health nursing theory and practice within a client and family centred model of care.
Demonstrated ability to provide client-focused, trauma informed, harm reduction, and culturally safe care.
Demonstrated ability to assess safety and risk, integrate data from multiple sources, and problem-solve effectively.
Demonstrated ability to manage psychosocial and behavioral issues and employ effective conflict resolution.

Responsibilities

Assess, plan, coordinate, organize, implement, evaluate and transition nursing and other services.
Assess service eligibility and facilitate the development, implementation and adaptation of care plans.
Deliver direct care and provide care management to assist clients in managing their own care.
Navigate clients through various community services and consult with other system partners.
Incorporate current evidence into practice and adhere to BCCNM standards.
