About this role

As a Community Health Nurse with Vancouver Coastal Health, you will care for clients with acute, chronic, and palliative care needs within an interdisciplinary team-based model. Working in partnership with clients and families, you will establish safe, realistic, and reasonable goals through comprehensive assessment, planning, and coordination of services.

Your day-to-day work involves assessing service eligibility, developing and adapting care plans, delivering direct care, and providing care management to help clients navigate community-based services. You will incorporate current evidence into practice and consult with other system partners to ensure seamless care transitions.

Requirements

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
One year recent related clinical experience in a relevant clinical area working with complex clients/patients with chronic health conditions and/or functional mobility issues
Recent experience in care and transition planning, or an equivalent combination of education, training, and experience
Valid BC Driver's License required as local area travel may require the use of a vehicle or other accepted mode of transportation
Demonstrated knowledge and skills in community health nursing theory and practice within a client and family centred model of care
Demonstrated ability to provide client-focused, trauma informed, harm reduction, and culturally safe care in assessments and interventions
Demonstrated ability to manage psychosocial and behavioral issues and employ effective conflict resolution and reconciliation approaches

Responsibilities

Assess, plan, coordinate, organize, implement, evaluate, and transition nursing and other services for clients with acute, chronic, and palliative care needs
Assess service eligibility and facilitate the development, implementation, and adaptation of care plans
Deliver direct care and provide care management to assist clients in managing their own care and navigating community services
Work in partnership with clients and families to establish safe, realistic, and reasonable goals
Consult with other system partners and incorporate current evidence into practice
Practice in accordance with BCCNM standards, Code of Ethics for Registered Nurses, and VCH policies and Decision Support Tools
Assess safety and risk prior to entering familiar and unfamiliar home environments
