About this role

Performs a variety of functions and technical procedures necessary for patient admission, assessment, diagnosis, treatment, follow-up, and outcome evaluation. Participates in performance improvement, research, and educational activities.

Day-to-day responsibilities include taking initial patient histories, performing physical examinations, and initiating the plan of care. Writes orders for medications, laboratory work, and diagnostic tests, and interprets results to guide treatment.

Confers with attending physicians, residents, nursing staff, and other care providers to ensure optimum quality of patient care. Performs daily patient rounds and manages discharge planning with prescriptions and referrals.

Opportunities to engage in performance improvement initiatives and educational activities. Collaborative practice agreements are supported under the Nurse Practitioner Modernization Act.

Requirements

  • Master’s Degree in a Nurse Practitioner Program and National Board Certification in specialty area required
  • Current license to practice as a Nurse Practitioner in New York State required
  • Current license to practice as a Registered Professional Nurse in New York State required
  • Collaborative practice agreement with designated protocols filed with NYS Department of Education or collaborative relationship attestation as per the Nurse Practitioner Modernization Act 2015
  • Current BLS certification required
  • Previous WHNP experience highly preferred

Responsibilities

  • Take initial patient histories and perform physical examinations
  • Initiate, document, and communicate the plan of care and follow up to ensure complete screening and preparation of patients
  • Perform daily patient rounds
  • Write orders for medications, laboratory work, and diagnostic tests
  • Interpret laboratory and test results
  • Confer with attending physicians, residents, nursing staff, and other care providers to ensure optimum quality of patient care
  • Note discharge summaries, prescriptions, and referrals on patient charts upon discharge
  • Inform patients of necessary post-discharge care instructions
