About this role
Galileo is a team-based medical practice working to improve the quality and affordability of health care for all. We are looking for a culturally-competent Nurse Practitioner to join our Population Health team, delivering personalized, comprehensive care to underserved patients and those with complex medical needs.
In this role, you will use phone and video-based care to provide high-quality follow-up to patients post-hospital discharge. Day-to-day work includes telephonic outreach and scheduled visits focused on medication reconciliation, symptom screening, and ordering follow-up labs or supplies.
You will navigate conversations to build trust, explain Galileo's care model, and connect patients to services. You will also coordinate care across settings — hospital and ED transitions, pharmacies, and community providers — to keep patients connected and moving forward.

Requirements
- Active Nurse Practitioner license in South Carolina
- Ability to work 32-36 hours per week
- Experience with phone and video-based patient care
- Strong ability to build rapport quickly, even on cold calls
- Curiosity and creativity in patient conversations
- Cultural competence in caring for diverse and complex patient populations
- Experience with transitions of care or post-discharge follow-up preferred
Responsibilities
- Cover care needs for our Transitions of Care program
- Conduct telephonic outreach and scheduled video/phone visits for recently hospitalized patients
- Perform medication reconciliation, symptom screening, and order follow-up labs, medications, or supplies
- Navigate conversations with patients to build trust and explain Galileo's care model
- Coordinate care across hospital and ED transitions, pharmacies, and community providers
- Collaborate with our nursing team
- Be accountable to individual performance targets while contributing to team-level population health goals
Benefits
- $78/hour regular rate and $90/hour premium rate
- Flexible 4-hour shift scheduling (mornings, afternoons, evenings, overnight)
