We are seeking a compassionate and skilled Psychiatric Nurse Practitioner to join our interdisciplinary team, providing high-quality psychiatric evaluation, diagnosis, and management for residents in skilled nursing facilities.
In this role, you will conduct comprehensive psychiatric assessments, diagnose mental health disorders, and develop individualized treatment plans. You will prescribe and manage psychotropic medications, monitor effectiveness and side effects, and adjust therapies as needed while collaborating closely with physicians, therapists, and nursing staff.
You will participate in care plan meetings, quality improvement initiatives, and value-based care programs aimed at enhancing psychiatric outcomes and reducing hospital readmissions. Your work will also involve educating residents, families, and facility staff on mental health conditions and behavioral interventions.

Requirements
- Master's or Doctorate degree in Nursing with specialization in Psychiatric-Mental Health Nurse Practitioner (PMHNP) from an accredited program.
- Active and unrestricted Advanced Practice Nurse (APN) license and certification as a Psychiatric Nurse Practitioner.
- Current DEA registration with prescriptive authority for controlled substances.
- Minimum of 2 years clinical experience preferred, especially in geriatrics, long-term care, or psychiatric settings.
- Strong knowledge of psychiatric disorders, psychopharmacology, and evidence-based mental health practices.
- Ability to work collaboratively within a multidisciplinary team and independently manage patient caseloads.
- Familiarity with electronic health records (EHR) systems and commitment to accurate clinical documentation.
- Must be licensed in both IL & IA.
Responsibilities
