This role focuses on assessing and evaluating the health care needs of patients and families, considering physiological, psychological, social, and environmental factors. The Community Health Nurse develops a care plan based on patient needs, physician orders, and nursing assessment parameters, ensuring goals respect patient, family, community, and company resources.

Day-to-day responsibilities include implementing and revising the care plan through regular assessment, observation, and evaluation of the patient's condition and response to care. The nurse initiates appropriate preventative, therapeutic, and rehabilitative nursing procedures, administers medications and treatments as prescribed, and performs nursing techniques.

Working within a Certified Home Health Agency (CHHA) environment, the nurse delegates responsibilities appropriately and supervises ancillary personnel to assure quality care and compliance with the care plan. The role requires completing and maintaining timely, accurate documentation to support patient care and regulatory standards.

This position offers the opportunity to apply clinical expertise in a community health setting, collaborating with patients, families, and interdisciplinary teams. It is ideal for a registered nurse with hospital or nursing home experience seeking to transition into home-based care.

Requirements

  • Current NYS Registered Nurse License
  • At least one year of hospital or nursing home experience
  • One year of community health experience in a Certified Home Health Agency (CHHA)
  • Strong clinical assessment and care planning skills
  • Ability to delegate and supervise ancillary personnel effectively
  • Proficiency in maintaining accurate and timely documentation

Responsibilities

  • Assess and evaluate the health care needs of patients and families, considering physiological, psychological, social, and environmental factors.
  • Develop a care plan based on patient needs, physician orders, nursing assessment parameters, and identified goals respectful of patient, family, community, and company resources.
  • Implement the care plan and revise it as necessary by regularly assessing, observing, and evaluating the patient’s condition, needs, and response to care.
  • Initiate and apply appropriate preventative, therapeutic, and rehabilitative nursing procedures and techniques.
  • Administer medications and treatments as prescribed by the physician and perform nursing procedures and techniques.
  • Delegate responsibility appropriately and supervise ancillary personnel to assure quality care and compliance with the care plan.
  • Complete, maintain, and submit all required documentation that is timely, accurate, and relevant.
