About this role

As one of the largest community health centers in the country, NeighborHealth is seeking a Home Care Nurse for our PACE Community Care program. This part-time weekend role provides direct and indirect nursing care, health maintenance, teaching, and planning for optimal patient functioning.

You will assess patients by obtaining history and performing appropriate tests and observations, communicate with providers, and educate patients on test results and care plans. You will also triage sick patients both on the phone and in the waiting room, demonstrating sound clinical judgment.

Working in a community health setting, you will collaborate with a multidisciplinary team to deliver comprehensive care. NeighborHealth serves diverse populations across greater Boston, and this position requires flexibility with evening, weekend, and on-call shifts with differentials.

Requirements

  • Completion of an Accredited Nursing Program.
  • Licensure in Massachusetts as a Registered Nurse (RN).
  • BCLS certification per department policy.
  • Bachelor of Science in Nursing preferred.
  • 1 year relevant nursing experience in a community health and/or ambulatory care setting preferred.

Responsibilities

  • Assess patients by obtaining patient history and performing appropriate tests and observations.
  • Communicate with the patient's provider and educate the patient on test results, new care plans, and other relevant health care information.
  • Demonstrate sound clinical judgment and decision making when triaging sick patients, both on the phone and in the waiting room.
  • Safely and effectively administer medications, tests, and treatments as ordered.
  • Teach patients and families health care regimens, explaining disease and wellness information, test results, medications, diet, exercise, and other plan of care aspects.
