CarePartners is one of Ontario’s largest accredited home health care providers, serving approximately 72,000 patients yearly in homes, schools, clinics, workplaces and retirement homes. Community nurses experience flexible scheduling and work-life balance in the home care setting. They develop meaningful therapeutic relationships with patients while preserving dignity and privacy.
This Temporary 6-month Full-Time position involves hours from 8 am to 8 pm in a 2-week rotation, including weekends, weekdays and on-call work in St. Thomas and surrounding areas. Nurses work within a supportive and collaborative environment focused exclusively on quality patient care. They exercise their full scope of practice and champion innovative health care programs.
Community nurses develop and maintain effective Care Plans independently or in cooperation with Ontario Health atHome. They provide exceptional nursing services to patients both in-person and virtually. The role emphasizes specialization and meaningful patient interactions in diverse settings like homes and clinics.

Requirements
- Registered Nurse (RN) or Registered Practical Nurse (RPN)
- Desire for great flexibility in schedules
- Ability to work independently in community settings
- Comfortable providing in-person and virtual nursing care
- Availability for 8 am - 8 pm shifts including weekends and on-call
- Access to reliable vehicle for patient visits
Responsibilities
- Develop and maintain an effective Care Plan with respect to Community Health Services, independently or in cooperation with Ontario Health atHome
- Provide exceptional nursing services to patients both in-person and virtually
- Build meaningful therapeutic relationships with patients in home care settings
- Preserve patient dignity and privacy during visits
- Exercise full scope of practice in community nursing
- Participate in 2-week rotations including weekdays, weekends and on-call duties
- Pursue specialization and champion innovative health care programs
