MindCare Solutions is a premier provider of behavioral health services, partnering with hospitals, emergency departments, inpatient behavioral health units, long-term care centers, correctional settings, and various other clinical settings. We are seeking a dedicated and compassionate Psychiatric Nurse Practitioner (PMHNP) to join our dynamic team on a full-time basis, focusing on geriatric patients in the LTC community.
In this role, you will collaborate with interdisciplinary teams to deliver high-quality, value-based psychiatric care across a variety of clinical settings. Your focus will be on proactive, evidence-based treatment strategies that improve the health and well-being of individuals while strengthening the capacity of partner organizations.
You will conduct comprehensive psychiatric evaluations, manage psychotropic medications, and develop personalized care plans in partnership with case managers and interdisciplinary teams. Regular team meetings to evaluate new referrals, review active cases, and adjust care plans are a key part of the collaborative environment.
This dynamic role emphasizes adaptability, innovation, and a patient-centered approach to behavioral health care. You will also contribute to quality initiatives such as performance improvement programs and medication reviews, with travel up to 50% of the time to meet with healthcare partners and support clinical settings.
Requirements
- Master's or Doctoral degree in Nursing from an accredited program specializing in Psychiatric Mental Health.
- Current and unrestricted PMHNP license in the state of Pennsylvania.
- Ability to collaborate effectively with interdisciplinary teams in a variety of clinical settings.
- Experience in geriatric psychiatry or long-term care settings preferred.
- Strong clinical assessment and diagnostic skills for psychiatric evaluations.
- Proficiency in evidence-based medication management and treatment planning.
Responsibilities
- Conduct comprehensive psychiatric evaluations to establish a baseline, manage psychotropic medications, and deliver personalized, need-based care.
- Develop and adjust treatment plans, including ongoing medication management, based on patient progress and input from the interdisciplinary team.
- Partner with case managers and interdisciplinary teams to create and refine care plans, ensuring a seamless, integrated approach to behavioral health management.
- Participate in regular team meetings to evaluate new referrals, review active cases, and adjust care plans based on patient progress and organizational goals.
- Contribute to quality initiatives such as performance improvement programs, medication reviews, and outcome optimization strategies.
- Travel up to 50% of the time to meet with healthcare partners, attend industry events, and provide consistent support across clinical settings.
