About this role

CareBridge Health, a proud member of the Elevance Health family within the Carelon business, is seeking Nurse Practitioners to serve as CareBridge Managers. This role enables individuals in home and community-based settings to maximize their health, independence, and quality of life through home care and community-based services.

In this virtual role, you will oversee and manage Clinical Programs operations while managing patient caseloads across multiple facilities. You will provide resources and direction to Nurse Practitioners, perform physical examinations, preventive health measures, and follow-up visits within prescribed guidelines and physician instructions.

You will order, interpret, and evaluate diagnostic tests to identify and assess patient clinical problems and healthcare needs, then discuss cases with physicians to formulate and document care plans. The role also involves prescribing medication or other forms of treatment and reviewing documentation to ensure compliance with best practices, protocols, and quality measures.

This position offers maximum flexibility and autonomy through a virtual work arrangement, with required in-person training sessions to support onboarding and skill development. The work shift is Monday through Friday, 8:30 am to 5:30 pm Eastern Time, with on-call scheduled once every two months.

Requirements

  • Requires an MS in Nursing
  • Minimum of 5 years of nursing experience; or any combination of education and experience which would provide an equivalent background
  • Requires an active, national NP certification
  • Requires valid, current, active and unrestricted Family or Adult Nurse Practitioner (NP) license in the state of Ohio, New York, New Jersey, or Massachusetts
  • Requires valid, current, active RN license
  • Possession of DEA registration or eligibility preferred
  • Experience in managing complex care cases for developmental disabilities and chronically ill patients strongly preferred
  • 1-2 years of leadership experience is preferred
