The Locum Paediatric Home Care Based Nurse provides compassionate, safe, ethical, patient- and family-centered care to paediatric patients. This includes comprehensive nursing assessments, patient advocacy, care coordination, medication administration, and supportive measures under clinical supervision. The goal is to enhance quality of life, accessibility, and continuity of care while improving outcomes.

Day-to-day work occurs in home settings, involving vital signs monitoring, IV cannulation, phlebotomy, and specimen handling according to program policies. Nurses monitor physiological, physical, and emotional responses, identifying early warning signs and seeking support. Collaboration with clinicians ensures care plans are adjusted based on patient responses.

The role supports patients and families through disease progression, symptom management, and advanced care planning. High expectations exist for professional services in this community-based environment. Opportunities arise to apply evidence-based practices in a rapidly expanding outreach network.

Requirements

  • Registration with Nursing Council of Kenya
  • Paediatric nursing background
  • Proficiency in comprehensive patient assessments
  • Skills in IV cannulation, access ports, and fluid administration
  • Experience with phlebotomy and specimen collection/handling
  • Knowledge of nursing process including assessment, diagnosis, planning, implementation, evaluation
  • Ability to monitor vital signs and recognize early warning signs
  • Home health or community nursing exposure

Responsibilities

  • Perform comprehensive patient and environmental assessments related to home health needs
  • Develop care plan in consultation with clinician
  • Coordinate and implement care delivery while anticipating patient care needs across the continuum, utilizing nursing process
  • Provide nursing care, administer medication and perform routine clinical procedures including vital signs, IV cannulation, phlebotomy in the home setting
  • Monitor the patient's physiological, physical and emotional response to treatment and identify early warning signs
  • Evaluate patient’s response to plan of care and collaborate with primary clinician to make adjustments
  • Support the patient and family through disease progression, symptom management, advanced care planning
  • Identify learning needs of the patient and families
